Cancer Genetic Risk Assessment Questionnaire The Cancer Genetic Risk Assessment Questionnaire (CGRAQ) was designed to reduce the duplication of medical histories taken by many of the physicians you may encounter in the course of your care. The CGRAQ is part of our clinical database. We are committed to protecting the privacy of our patients. We want to provide a safe and secure user experience for you. We will use our best efforts to ensure that the information you submit to us remains private, and is used only for the purposes as set forth herein. The following reflects our commitment to you. This notice applies to all information collected or submitted on this website.

General Information Disclosure Except as described in this privacy policy, we do not disclose information about your individual visits to our website, or personal information that you provide, such as your name, address, e-mail address, telephone number, social security number, personal references, etc., to any outside parties, except when we believe it is required by law or to protect our users or others. But, we may record and share aggregated information with other physicians and medical personnel who are involved in your care. Also, we may employ other individuals to perform functions on our behalf. Examples may include hosting our web servers, analyzing data, updating our clinical database, processing payments, and providing customer service. These other individuals will have access to your personal information as necessary to perform their functions, but they may not share that information with any third party. Since CGRAQ is personal and sensitive information, we give you options in entering your information in our database. There are two ways of doing this: 1. You can store your health information in our clinical database, and allow it to be searchable by your physicians and medical personnel concerned with your care. We use our best efforts to grant access to this database only to doctor(s), and other professionals involved in your care. Our institutional policies prohibit release of any individuals' information to anyone or the use of any confidential health information other than when required by law. 2. You can enter part of your information and stop, take a break, or even close the website. At a later time when you come back to the website, you will have to use your Medical Record Number(MRN) and your password to access your already entered information to complete the questionnaire, or to edit any information before you finally submit the completed form permanently to our clinical database.

Updating Questionnaire Information The CGRAQ allows you to change or correct your personal health information at any time. To do so, simply log into our Cancer Genetic Risk Assessment Questionnaire website, then start to enter or modify your information.

Privacy Commitment Changes If we decide to change our privacy commitment for CGRAQ, we will post those changes here so that you will always know what information we gather, and how we might use that information, and whether we will disclose it to anyone.
